CoQ10 appears on a lot of supplement labels and is explained on almost none of them. It sits there, impressive and abbreviated, doing unspecified work.
So here is the whole thing: what coenzyme Q10 actually is, what the trials have and have not shown, when to take it, how much, what it clashes with, and who probably does not need it at all.
The short version
What it is: a fat-soluble compound your body makes itself, used inside mitochondria to produce cellular energy, and acting as an antioxidant in cell membranes.
Typical dose: 100 to 200 mg daily in most supplement use. Food gives you only around 5 mg a day.
Best time: with a meal containing fat. This alone can raise absorption several-fold. Earlier in the day, not at bedtime.
Main caution: if you take warfarin, speak to your doctor before starting. This is not a formality.
What is CoQ10?
Coenzyme Q10, abbreviated to CoQ10 and also called ubiquinone, is a fat-soluble compound found in essentially every cell in your body. The name is a clue. Ubiquinone comes from ubiquitous, because it is everywhere.
It does two distinct jobs.
First, it is part of how your cells make energy. Inside mitochondria, the electron transport chain shuttles electrons along a series of steps to produce ATP, the molecule your cells actually spend as energy. CoQ10 is one of the carriers in that chain. Without it, the chain does not run. This is a description of biochemistry, not a claim about how you will feel.
Second, it is a fat-soluble antioxidant. Because it dissolves in fat, it works inside cell membranes and lipoproteins, where water-soluble antioxidants like vitamin C cannot reach.
Your body synthesises CoQ10 itself, which is why it is not classified as a vitamin. But two things reduce it. Levels decline with age, and statin medications measurably lower circulating CoQ10, because statins block the mevalonate pathway, which the body uses to make both cholesterol and CoQ10. That is a well-documented, mechanistically obvious consequence.
Diet contributes very little. CoQ10 is found in organ meats, oily fish and beef, but total dietary intake is typically only around 5 mg per day. A supplement dose of 100 mg is therefore roughly twenty times what food provides, which is the honest reason supplementation exists as a category at all.
Benefits of CoQ10: what the research actually shows
This section is written carefully, and the care is the point.
CoQ10 has no approved health claim. It is not a vitamin with an authorised function claim, and it is not approved to treat any condition. Anyone telling you CoQ10 boosts your energy is making a claim that regulators have specifically declined to authorise. So rather than tell you what it will do for you, here is what specific trials found in specific populations.
- Heart failure. The strongest evidence comes from the Q-SYMBIO trial, which used 300 mg daily of ubiquinone in patients with chronic heart failure and reported a significant reduction in major adverse cardiovascular events. Note the population. These were patients with diagnosed heart failure, under medical care, not healthy adults looking for a lift.
- Migraine prevention. Most randomised trials in this area used 100 to 300 mg daily, and CoQ10 appears in some migraine prevention guidance as an option.
- Statin-associated muscle symptoms. This is the most requested use and, awkwardly, the least settled. Statins do deplete CoQ10, that part is clear. Whether replacing it relieves the muscle aches is not. Trial results have been inconsistent, and current cardiology guidance does not recommend routine CoQ10 use for statin-related muscle symptoms, on the grounds of unproven benefit rather than any safety concern. Some people report improvement. The evidence does not yet support promising it.
- Blood pressure and metabolic markers. Meta-analyses in cardiometabolic populations have examined effects here, with modest findings and no significant safety signal across trials.
What you should take from this: CoQ10's evidence is strongest in clinical populations and weakest in healthy people looking for more energy, which is precisely the reverse of how it is usually marketed.
Best time to take CoQ10
This is the highest-leverage, lowest-effort thing on this page, and most people get it wrong.
Take it with a meal that contains fat
CoQ10 is fat-soluble and notoriously badly absorbed. Estimates of oral bioavailability run somewhere in the range of a few percent of the dose. But studies consistently show substantially higher plasma CoQ10 when it is taken with food rather than fasted, with reported increases of roughly two to four-fold.
Read that again, because it is remarkable. Swallowing your CoQ10 with breakfast rather than on an empty stomach can do more for your actual CoQ10 levels than paying extra for a premium form. It costs nothing. A meal with some olive oil, nuts, eggs, curd, ghee or oily fish is enough.
Earlier in the day, not before bed
Some people find CoQ10 mildly stimulating, particularly at higher doses, and report difficulty sleeping if they take it late. There is no strong trial evidence for this, but it is commonly enough reported that the sensible default is breakfast or lunch rather than dinner. If you sleep fine on it, this does not apply to you.
Split doses above 100 mg
If you are taking more than 100 mg a day, dividing it across two or three meals is associated with fewer digestive complaints and may help absorption, since the absorption pathway appears to saturate.
CoQ10 vs ubiquinol: which form should you buy?
This is the argument the supplement industry most wants to have with you, because ubiquinol costs more.
CoQ10 exists in two states. Ubiquinone is the oxidised form. Ubiquinol is the reduced form. Your body converts between them constantly, and both end up doing the same jobs.

The honest summary: formulation matters at least as much as form. A well-made ubiquinone softgel in an oil base will typically outperform a poorly made ubiquinol powder capsule. Particle size, oil carrier and delivery system are doing a lot of the work that gets credited to the ubiquinol label.
And remember what the previous section said. Taking a cheap ubiquinone with a fatty meal will very likely serve you better than taking an expensive ubiquinol on an empty stomach.
Can you combine CoQ10 with probiotics?
Yes. There is no known interaction between CoQ10 and probiotic strains, and no reason to expect one. They operate through entirely unrelated mechanisms in different parts of the body: probiotics act in the gut lumen, CoQ10 is absorbed and works inside your cells.
Two practical notes if they are in the same product or taken together:
- The fat requirement still applies. CoQ10 needs dietary fat for absorption regardless of what it is sitting next to. Combining it with probiotics does not change that.
- Check what else is in the blend. The relevant question with any combination product is never really about interaction. It is about dose. A formula containing CoQ10 at 5 mg alongside twenty other ingredients is not delivering a meaningful amount of CoQ10, whatever the label implies.
The same applies to combining CoQ10 with a multivitamin, with fibre, or with most other supplements. The genuine cautions are with medications, not with other nutrients.
Side effects of CoQ10
CoQ10 is generally well tolerated, with a good safety record across clinical trials at doses from 100 mg to several hundred milligrams daily. Where side effects occur they are usually mild.
- Nausea, upper abdominal discomfort or heartburn
- Loss of appetite
- Diarrhoea
- Headache or dizziness, less commonly
- Difficulty sleeping if taken late in the day
- Rash, rarely
Digestive side effects are often reduced by splitting doses above 100 mg across the day and taking them with food.
Drug interactions, read this properly
Warfarin. This is the interaction that matters. CoQ10 is structurally similar to vitamin K, and warfarin works by blocking vitamin K-dependent clotting factors. CoQ10 may therefore reduce warfarin's effect, and cases of INR falling below the therapeutic range have been reported. If you take warfarin, do not start CoQ10 without speaking to your doctor or anticoagulation clinic first. It may still be possible with monitoring, but it must be coordinated, not improvised.
Blood pressure medication. CoQ10 may have a mild blood pressure lowering effect, which could be additive with antihypertensives. Worth mentioning to your doctor.
Diabetes medication. Some research suggests CoQ10 may affect blood glucose, so anyone on glucose-lowering medication should monitor and inform their doctor.
Pregnancy, breastfeeding and children. Safety has not been well established in these groups. Do not use without medical advice.
How much CoQ10 do you need daily?
There is no recommended dietary allowance for CoQ10, because your body makes it. It is not an essential nutrient in the way iron or B12 is, and you cannot be clinically deficient in it in the ordinary sense.
What exists instead are the doses used in research:

The number that should concern you most is not on that table. It is the number on the label of the product you are actually holding. Combination formulas frequently include CoQ10 at 5 or 10 mg, an amount chosen so the ingredient can appear on the front of the box rather than because it does anything. Every trial cited on this page used at least ten times that.
Check the dose per serving. If it is not there, that is your answer.
Who should actually consider CoQ10?
Reasonable candidates, based on the evidence rather than the marketing:
- People taking statins. The depletion is real and well documented, even if the benefit of replacing it is contested. Discuss with your doctor.
- Adults over 50 or 60, since natural levels decline with age.
- People with diagnosed cardiovascular conditions, where the evidence is strongest, and always under medical supervision rather than self-directed.
- People exploring migraine prevention, in discussion with a doctor.
And the honest counterpart. If you are a healthy adult under 40 with no medications and no specific condition, the evidence that CoQ10 will make you feel different is weak. You may well be better served spending the money on things with clearer evidence and a clearer deficiency behind them, such as vitamin D or iron, both of which you can actually test for.
